Alzheimer’s disease (AD) pathophysiology seldom occurs alone, and the presence of multiple intersecting pathologies is the largest predictor of dementia1-4. A major gap in the field is the ability to identify multi-etiology causes of impairment and antecedent states in-vivo. The Alzheimer’s Disease Research Centers (ADRCs) are uniquely positioned to meet this known but unaddressed need. Collectively the ADRCs longitudinally follow ~14,000 participants with high brain donor and autopsy rates (each >60%).
The ADRCs recruit across the clinical severity continuum and amply represent the several diseases comprising AD and Related Dementias (ADRD). But there is a major resource gap as in vivo imaging and plasma collection on this multi-site cohort are ad-hoc, and not uniform. As a consortium, the ADRCs will conduct a standardized imaging and plasma protocol that profiles AD (Amyloid and Tau), Vascular ischemic burden, and Neurodegeneration signatures of ADRD on expertly characterized patients.
We propose a longitudinal study at ~2-year intervals superimposed on the ongoing Clinical Cores of the ADRCs; we will study 2000 participants that are either clinically unimpaired (CU; N ~800) or impaired (mild cognitive impairment [MCI] or dementia; N ~1200) where AD is among the considered etiologies but need not be primary.

Eligibility Criteria
The total sample size is 2,000. Approximately 800 (40%) will be cognitively unimpaired and approximately 1200 (60%) will have MCI or dementia at baseline CLARiTI visit. AD is among the possible etiologies but need not be primary and the participant does not need prior biomarker confirmation to enroll.
An ADRC Clinical Core participant with NACC ID
Age at Enrollment: Participant age is 50 years or greater.
Cognition: Cognitive stage is unimpaired, MCI or dementia (mild to moderate at the discretion of the site). If cognitively unimpaired, the participant meets any one of these high priority characteristics: URP
Broadly defined; At higher risk for all cause dementia Less education 12 or fewer years APOE4+ Higher risk for amyloid+ and earlier age of A+ onset First degree family history of dementia From a neurodegenerative disease but need not be AD. This construct denotes non-specific genetic and environmental risk Vascular health factors 1 or more of: HTN, cholesterol, prediabetes, diabetes, heart disease, current smoker, obesity, sleep apnea, in the judgement of the site Cognitive complaints Subjective complaints are documented in UDS forms. Complaints may be from participant or informant.
Cognitive decline
Transitional decline based on judgment of the site team from collected history or from observed testing. Decline is not sufficiently severe to meet criteria for MCI Age is 70 or greater Age is the greatest risk factor for all cause dementia Neurobehavioral features REM-BD, MBI or other neurological features.
