Yale Medicine is renowned for its excellence in geriatric care, focusing on helping older adults maintain independence and a high quality of life while managing complex health conditions. Our multidisciplinary team, composed of geriatricians, geriatric psychiatrists, nurse case managers, patient care assistants, physical therapists, and neuropsychologists, collaborates closely with patients, families, primary care providers, and other health care professionals to develop comprehensive care plans.
Our physicians have pioneered widely adopted interventions and provide crucial support to families dealing with functional decline, disability, and decision-making challenges. We are home to one of the most comprehensive geriatric assessment centers in the country, the Dorothy Adler Geriatric Assessment Center, where assessment and evaluation is available for older adults who are struggling to take care of themselves.
