Our geriatric care team is dedicated to helping older adults stay independent and enjoy the best quality of life possible, even when dealing with complex health issues. Our care team includes geriatricians, geriatric psychiatrists, nurse case managers, patient care assistants, physical therapists, and neuropsychologists.
Our goal is to support you and your family every step of the way. We work closely with you, your family, and your primary care doctor to create a personalized care plan tailored to your needs. We’ve developed practical solutions for the everyday challenges aging can bring, such as dealing with functional decline, disability, problems with decision-making, and even driving issues.
Indicators of functional decline in older adults include:
- Decreased ability to perform daily tasks
- Loss of independence
- Difficulty with routine activities
- Slowing down physically
- Challenges with movement and coordination
- Reduced physical and mental abilities
- Gradual weakening of skills and abilities
Common aspects of decision-making difficulties include slower decision processes and decreased confidence, memory challenges, reduced focus, and overwhelming feelings of anxiety and depression.
Yale Medicine is home to one of the most comprehensive geriatric assessment centers in the country, the Dorothy Adler Geriatric Assessment Center, where assessment and evaluation is available for older adults who are struggling to care for themselves.
