This study is evaluating community-based approaches to help control high blood pressure, including remote blood pressure management with or without support from a community health worker, compared with standard screening and referral to primary care.

Participants will have their blood pressure confirmed through screening measurements and, if eligible, will receive a home blood pressure monitor that automatically sends readings to a blood pressure management clinician. Participants will take regular blood pressure readings at home and may receive additional support from a community health worker, depending on study assignment.

    Eligibility Criteria

    Who May Participate:

    Adults age 18 or older with elevated blood pressure (average resting BP ≥135/85 mmHg) identified through community screening.

    Not Eligible:

    Individuals who are pregnant, planning pregnancy within 6 months, receiving dialysis for end-stage kidney disease, or undergoing active cancer treatment are not eligible.
