Daniel Jacoby, MD, is director of the Comprehensive Heart Failure Program which includes the Center for Advanced Heart Failure, Heart Failure Population Health, and the Cardiomyopathy Program. He specializes in the diagnosis and treatment of heart failure. Dr. Jacoby is recognized internationally as an expert in hypertrophic cardiomyopathy.

Dr. Jacoby is dedicated to serving the needs of patients and their families, from initial diagnosis through all stages of treatment. “There’s no way you can be a good provider unless you care about people,” Dr. Jacoby says. “When a physician walks out of a patient’s room, both the doctor and the patient should feel a little better.”

In 2009, he founded cardiomyopathy program to provide expert care for patients with inherited and infiltrative cardiomyopathy. Dr. Jacoby has been director of the Comprehensive Heart Failure Program since 2014.  

As an associate professor of internal medicine at Yale School of Medicine, Dr. Jacoby conducts research into general heart failure, as well as hypertrophic, dilated, and arrhythmogenic cardiomyopathy. He enjoys the opportunities to provide long-term follow-up care for patients and their families.
