Mental Health Collaborative Care Program

The Yale Medicine Mental Health Collaborative Care Program was created to provide a proven team approach to help patients with depression and offer them ongoing outpatient care.  

We believe in letting patients actively participate in their care. Patients can expect regular check-ins and monitoring of their symptoms. We also provide patients with skills to help them feel better and activities they can practice on their own time.  

Our care team includes primary care providers, a depression care manager, and a psychiatric consultant.
